Bugaboo Car Seat adapter 

Your baby's car seat is supposed to keep him safe, but if you are using a Bugaboo Car Seat adapter with the model number 80400GC01 or 80401GC02 to anchor your baby's seat into a stroller, stop using the adapter, and contact the company's service department at serviceus@bugaboo.com or (800) 460-2922 between 7 a.m. and 4 p.m to receive your free service kit and decals with proper use instructions. On certain types of strollers, the car seat can disconnect from the stroller when in the forward-facing position.
